You can request the results of any tests you may have had such as blood tests or x-rays (if ordered by the surgery) by completing the form below. If you have had tests ordered by the hospital we recommend that you contact them first. Please leave 2 weeks after testing to enquire to ensure all results are back. You may be asked to make an appointment to discuss your results with your doctor also. We will email you a copy of the results.
